Make a difference in the lives of children and families by providing skilled, compassionate care in the comfort of their homes. As a Pediatric Homecare Nurse, you’ll play a vital role in helping young patients thrive in a supportive, family-centered environment.
The Pediatric Registered Nurse Case Manager provides skilled nursing assessments and care to children with complex medical needs. We support our nurses with strong clinical guidance and a nurturing team culture.
Performance Responsibilities and Standards:
The Pediatric Homecare Nurse is responsible for providing compassionate, high-quality care to children and supporting their families in the home setting.
- Deliver safe, high quality, safe and compassionate nursing care to pediatric patients and families in the home setting.
- Serve as the primary case manager coordinating all aspects of the child’s care with the interdisciplinary team.
- Develop and implement individualized plan of care.
- Ensure documentation meets all quality and regulatory standards.
- Educate and support families to promote optimal outcomes.
- Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care.
- Adhere to all Nascentia Health policies, procedures, and quality initiatives, always maintaining patient and employee confidentiality.
- Demonstrate knowledge of and compliance with infection control practices, as well as federal, state, and local regulations.
- Must adhere and be knowledgeable of all organizational quality improvement standards.
- Maintain and update clinical knowledge on an on-going basis.
Job Qualifications:
- Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
- Current and valid driver's license and auto insurance, and a vehicle which can be used for work (we do have a Fleet Program for field clinicians).
- Self-directed and organized with the ability to work independently in a fast paced environment.
- Previous pediatric and/or MCH experience a plus
- Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
- Exceptional oral, written and verbal communication skills.
Physical Requirements:
- Ability to sit or stand at a desk 90% of the day
- Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
- Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
- Visual/hearing ability sufficient to comprehend written or verbal communication
- Ability to use telephone and communicate over the phone
- Frequently uses a laptop computer to enter orders and obtain product information
- Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
Compensation & Benefits:
Competitive Salary [This position is an S03 exempt position with a min-max rate of: $75,250- $87,138.92 /annually] with:
