Position Description:
The Physical Therapist Assistant (PTA) provides care to patients with health conditions that may limit their ability to move and perform daily activities, under the direction and supervision of a licensed Physical Therapist (PT). The PTA assists in implementing treatment plans designed to restore physical function and mobility, promote wellness, and improve patients’ quality of life. The PTA delivers skilled therapy services in alignment with the mission and objectives of Nascentia Health.
Performance Responsibilities and Standards:
1. Ensure quality and safe delivery of home health services.
- Provide safe Physical Therapy interventions to clients/families in accordance with agency policies and procedures.
- Provide treatment in accordance with Physical Therapy plan of care using best clinical practice.
- Perform intermittent assessments as directed by physical therapist supervisor.
- Maintain patient care records using approved documentation and format.
- Documentation reflects care and interactions.
- Documentation is completed in a timely manner.
- Documentation is clear & concise. Including communication between PT and PTA.
- Follow professional code of ethics and standards of practice.
- Manage a diverse patient caseload.
- Communicate patient care concerns to interdisciplinary team including nursing, speech, and occupational therapy.
- Conduct environmental and safety assessments, providing recommendations for durable medical equipment, transfer aids, grab bars, lift chairs, and other adaptive or supportive modifications to enhance clinician, patient, and caregiver safety, mobility, and independence.
2. Assume responsibility for supervision of paraprofessionals relevant to patient care plan
- Monitor job performance of home health aides and delivery of home health aide services.
3. Comply with Nascentia Health policies and procedures.
- Maintain Nascentia Health productivity standards.
- Maintain adherence to agency policies and procedures including Corporate Compliance, Ethical Standards, and Confidentiality (HIPAA).
- Demonstrate knowledge of federal, state and local government rules and regulations.
- Demonstrate knowledge and use of infection control practices.
- Adhere to dress code policy.
- Adhere to quality improvement standards.
- Keep personnel requirements up to date, i.e. health requirements, professional license requirement, and other Nascentia Health requirements.
- Demonstrate skills competency through periodic evaluations.
4. Participate in personal growth and development.
- Participate in education in-services and meetings.
- Maintain and update clinical knowledge on an ongoing basis.
5. Present a professional image to clients and the community.
6. Perform all other job duties as assigned.
Requirements:
- The applicant must be currently licensed and registered as a Physical Therapist Assistant by the New York State Education Department.
- Current New York State driver’s license and automobile insurance.
- One year of experience as a Physical Therapist Assistant preferred but not required.
- Applicant must be in good standing and not under any current disciplinary action by a licensing or regulatory authority.
Physical Requirements:
- Sufficient speaking/visual/hearing to comprehend and express written/verbal communication.
- Ability to perform tasks involving physical activity including:
- Pushing/pulling - Typical weight 50 pounds / Maximum weight 150 pounds
- Lifting/ carrying - Typical weight 25 pounds / Maximum weight 40 pounds
- Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g., snow, rain, sleet, and going up and down stairs.
- Ability to drive during all weather conditions.
- Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

Competitive Salary
This is a per-visit position with an estimated annualized range of $68,250–$85,000+, based on visit volume*.
*Actual compensation will vary depending on the number of completed visits per pay period. A minimum average of 70 visits per pay period is required to maintain full-time status.
