Position Description:
The Care Coordinator Supervisor (CCS) provides administrative, technical and leadership support to the Care Coordinators and clinical teams. Responsible for overseeing the operational, quality and process improvement aspects of the Care Coordinator role.
Performance Responsibilities and Standards:
- Participates in interviews, hiring, performance evaluations
- Provides supervision to Care Coordinators and Authorization Coordinators for all branches
- Coordinates with Senior Care Coordinator/Education Specialist to provide orientation and training of departmental staff
- Acts as a resource to team
- Ensures compliance with all MLTC regulations and contractual obligations
- Establishes and monitors quality of work provided by staff and takes appropriate action to make improvements as appropriate
- Works with Senior Care Coordination/Education Specialist to update the Care Coordinator training manual and integrates changes based on MLTC policies and process.
- Provides ongoing training, support and mentoring to staff as needed.
- Facilitate the development of an integrated Care Coordinator role supporting team-based interventions.
- Assists in delegating responsibilities to cover for Care Coordinators that are out of office.
- Responsible for overseeing accurate review and entry of authorization data into computerized database.
- Assists with troubleshooting authorization issues.
- Completes Quality Audits to assure that Care Coordinator responsibilities are met.
- Actively participates and oversees performance improvement and continuous quality improvement (CQI) activities.
- Participates in MLTC Leadership activities and meetings.
- Provides feedback to MLTC Leadership team when necessary.
- Provides triage of phone calls that are escalated due to concerns or complaints. Escalates issues as appropriate.
- Meets regularly with Care Coordinators and clinical team and makes recommendations for process improvement.
- Completes Care Coordinator Audits for annual reviews, coaching plan and/or PIP’s
- Plans, coordinates and hosts Care Coordinator team meetings.
- Manages various reports within EHR as directed.
- Maintains appropriate member case load, as indicated.
- Performs all other duties as assigned.
Requirements:
- Associate degree in related field, or equivalent experience. Bachelor’s degree preferred.
- 3-5 years’ experience in the healthcare industry or a degree as Medical Assistant.
- Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.
- Proficiency with computer systems, working knowledge of Microsoft Office, Internet and various software applications.
- Demonstrate ability to work independently and collaboratively with ability to adapt to shifting priorities, demands, and timelines.
- Demonstrates discretion in handling confidential and sensitive information
- Excellent communication skills, verbal and written.
Physical Requirements:
- Speaking/visual/hearing ability enough to comprehend and express written/verbal communication.

- Ability to sit or stand at a desk 90% of the day
- Pushing, pulling, reaching, kneeling
