About the Role!

The Speech Therapist provides comprehensive, professional speech-language pathology services in alignment with the mission and objectives of Nascentia Health. This role is responsible for delivering safe, effective therapy interventions to patients and families in their homes, in accordance with physician orders, agency policies, and professional standards of practice.

This is a field-based position serving the Utica, Rome, Oneida, Remsen, Camden, Sylvan Beach, and Clinton areas.

What You’ll Do!

Provide high-quality, patient-centered speech therapy services that support safety, function, and independence.

Key Responsibilities Include

  • Conduct comprehensive patient assessments using best clinical practices
  • Develop and implement individualized plans of care
  • Deliver safe and effective speech therapy interventions in the home setting
  • Maintain accurate, timely, and compliant clinical documentation, including OASIS requirements
  • Manage a diverse patient caseload while ensuring quality and continuity of care
  • Communicate patient status, progress, and care needs with the interdisciplinary team
  • Supervise and support paraprofessionals (e.g., home health aides) as it relates to the patient care plan
  • Monitor and evaluate the quality of home health aide services
  • Ensure adherence to all agency policies, procedures, and regulatory requirements (including HIPAA and infection control practices)
  • Participate in case conferences, team meetings, and quality improvement initiatives
  • Maintain productivity standards and professional competency requirements
  • Engage in ongoing professional development, including in-services and continuing education
  • Represent Nascentia Health with professionalism in all patient and community interactions
  • Perform other duties as assigned

What We’re Looking For!

  • Current New York State license as a Speech-Language Pathologist required
  • Registration with the New York State American Speech-Language-Hearing Association (ASHA) and educational standard board as applicable
  • Minimum of one year of experience as a Speech Therapist preferred; home care experience a plus
  • Strong clinical assessment and treatment planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently in a field-based setting while collaborating with a team
  • Strong organizational and documentation skills
  • Knowledge of federal, state, and local healthcare regulations
  • Current New York State driver’s license and reliable transportation with automobile insurance required

Physical Requirements

  • Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
  • Ability to perform tasks involving physical activity including:
  • Pushing/pulling -typical weight 50 pounds / maximum weight 150 pounds
  • Lifting and carrying-typical weight 25 pounds / maximum weight 40 pounds
  • Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, e.g. snow, rain, sleet and going up and down stairs.
  • Ability to drive on a daily basis in all weather conditions throughout the working year
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues)
