Position Summary: Participates and facilitates the discharge planning process and initial determination of home care needs for patients in all age groups who are referred for home care services. Meets with discharge planners at referring facilities to promote referrals to Oswego Health Home Care and affiliated services. Assist in day-to-day operations of the Home Care Office, patient enrollment and education to Oswego Health’s Virtual Senior Center, and assists in the facilitation of quality patient care.
Duties/Responsibilites:
Education, Training, Experience, Certification and Licensure: Experience in home care with specific knowledge of the intake process. Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise.
Special Equipment, Skills, or Other Requirements: Demonstrates interest and aptitude in community home health. Demonstrates ability to make sound judgments without close supervision. Has excellent communications skills. Possesses knowledge of documentation for services rendered in acceptable format for third party payers. Must have valid driver license and reliable personal vehicle.
Work Environment and Hazards: Clinical and office setting. OSHA category I.
Physical Demands: Light work: standing, walking, sitting for prolonged periods of time, repetitive motions of eyes, hands, fingers and wrists with writing or data entry. Input data on keyboard, requires visual acuity.
Pay Range; $500/week with monthly bonus structure for generated referrals.
Employee salary is based on commensurate experience and other qualifications.
