REPORTS TO DIRECTOR OF CLINICAL QUALITY
BROAD FUNCTION:
Responsible for planning the scope and carrying out the plan for the hospital social service program in accordance with the identified needs of the hospital and accepted social work practice.
MAJOR RESPONSIBILITIES:
1. Use high risk screening criteria, conduct psychosocial assessments, develop, evaluate, implement and coordinate appropriate admission or discharge plans to meet the needs of the patient with full participation of the patient and family, as well as the goals of the hospital. Makes referrals to Discharge Planning, Nursing Staff and other appropriate agencies as deemed necessary.
2. In conjunction with the Discharge Planner provides for discharge planning and assessment of patients’ needs appropriate to the age of the patient (including completing screens).
3. Meets with patients or representatives who are referred to review the Patient Rights/Advanced Directives and to provide information on how to provide information on how to exercise these rights/advanced directives.
4. Provides short term counseling for patient/family with social, emotional and family problems precipitated by illness and hospitalization.
5. Assists the health care team in understanding the significance of social, emotional, economic and family factors in relation to the patient’s illness, treatment and recovery.
6. Conducts Mental Health Evaluations on Adult and Pediatric Patients.
7. Coordinates referrals to the NY State Registry for child abuse/maltreatment and or adult protective services through local DSS and law enforcement agencies.
8. Assists with Social needs such including but not limited to Emergency Department frequent users, patients wishing to leave AMA, assisting patients/families adjusting to their illness, patients in need of concrete supportive services such as housing, transportation, and home care, those in need of short term grief/bereavement counseling, and coordinates newborn adoptions.
9. Develops and carries out departmental performance improvement program.
10. Demonstrates competency in the management of the environment including infection control, utilities management, life safety, emergency preparedness, hazardous materials and waste management, security management and patient safety goals.
11. Develops and maintains referrals and resource listings of community agencies. Serves as a liaison between patient and community resources including but not limited to homelessness and substance abuse.
12. Assists patient in getting financial assistance when required and referring to appropriate community agencies.
13. Demonstrates competency in the ability to interact with adolescents, adults and geriatric patients.
14. Accepts other assignments and responsibilities as necessary.
15. Demonstrates understanding of and compliance with pain management policies/protocols.
16. Demonstrates understanding of an compliance with safety management policies/protocols.
17. This position will require access to PHI (Protected Health Information). Electronically: ABS, PCI, MRI. Written/Paper: Reg. Info, H&P, X-ray, Labs, Progress Notes, MD Orders, Report of Operation, Med Sheets, Nursing Notes, Other. Oral: YES Ensures adherence to all HIPAA policies and procedures.
18. Collaborates with medical staff, nursing, Administration and other hospital personnel as well as community partners, patients and family members.
19. Promotes confidence and trust through open and direct communication.
20. Consistently maintains confidentiality.
21. Keeps others informed as to pertinent issues.
22. Documents activities thoroughly in medical records, screens, etc.
23. Seeks consultation, references, and/or resources when necessary.
24. Provides staff orientation/education related to social services.
25. Acts as a patient advocate at all times.
26. Abides by the National Association of Social Worker’s Code of Ethics.
27. Demonstrates effective and efficient organizational skills.

29. Insures economical, appropriate and safe use of supplies and equipment at all times.
30. Attends staff and inservice meetings and hospital-wide inservice program.
31. Participates in continuing education related to Social Services.

WORKING CONDITIONS:
Stands, walks up to half the day. Works most of the day in patient care units.
QUALIFICATIONS:
Must have a Master’s Degree from an approved School of Social Work with NYS licensure or permit eligible, experience in substance abuse and two years medical social work preferred. This position provides service and demonstrates the knowledge and ability to meet the specific needs of all patient age groups. Previous experience with children preferred.
Pay Range: $31.50-$36.50/hr
Employee salary is based on commensurate experience and other qualifications.
