Interprets medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Pharmacist prepares compounds and dispenses medications for inpatient and ambulatory care use. This includes the preparation of injectable sterile products. Dispenses Pharmacy related items. Performs duties in all areas of Pharmacy to include inpatient, ambulatory care and satellites. Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Pharmacy Departments Quality Assurance Programs.
This pharmacist will primarily provide pharmacy services at Upstate’s Cancer Center and as needed, other associated Upstate oncology clinics located throughout the Central New York Region. The Upstate Cancer Center and affiliated clinics provide state of the art cancer care to the patients it serves. Under the direction of the hospital’s Pharmacy Department, the Cancer Center Pharmacy functions via a dedicated pharmacy space with its own modern sterile products laboratory. The applicant will work with a team of oncology-trained pharmacists and technicians to manage the preparation and oversight of oncology treatments. This includes, but is not limited to, patient profile review, sterile and non-sterile medication validation/preparation, patient counseling, formulary/inventory management, and staff/provider communication.
B.S. in Pharmacy or Pharm.D., NYS license and registration required; completion of PGY1 Pharmacy residency or equivalent experience required.
Previous hospital experience preferred. Previous pharmacy oncology experience or completion of a PGY-2 Oncology Residency preferred. Previous pharmacy sterile products experience preferred.
