Job Summary:
The Social Worker - Community Based Organization Coordinator assesses patient and organizational needs and collaborates with Community-based Organizations (CBO's) in the development of strategies and programs to address identified patient and community needs. The SW - CBO Coordinator will have extensive knowledge of local, regional and national Community-based Organizations, have strong relationships with the CBO's and will be the primary contact for Community-based agencies who are partners in our patient's care. The Social Worker - CBO Coordinator will build and strengthen relationships with CBO partners, ensure accountability, strengthen communication, make appropriate connections, and facilitate communication between agencies and clinics. Will also build new programs and develop collaborative strategies to address the needs of our patient population. The SW - CBO Coordinator will facilitate in the connection of individual patients with CBO's designed to meet their needs in a patient-centered manner. Through the use of a wide range of interventions and treatment modalities, will contribute to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care.
Minimum Qualifications:
MSW from a Social Work program accredited by the Council on Social Work (CSWE) required. NYS Certification (LMSW) and a minimum of five years of post-Master's experience in a Transitional Care program required.
Preferred Qualifications:
Certification in Care Coordination and Transition Management preferred.
Work Days:
Monday - Friday 8am - 4:30 pm
