Working within a multi-disciplinary team, will provide health care coordination services for person living with HIV/AIDS, and LGBTQ population with focus on patients with a mental health diagnosis. Position will ensure retention of clients in medical and mental health care by advocating for identified and needed services, addressing barriers to care, and ensuring the maintenance of on going services. The goal of the program is to see improvement in the overall functioning of the patient, leading to improved self-efficacy.
Associates degree (preferably in business or health related field), 3 years relevant experience plus excellent written/oral communication, organizational and interpersonal skills required.
Bi-Lingual English/Spanish. experience working in a medical clinic with persons living with HIV/AIDS, and knowledge of regional programs and services preferred
